Compliance (or adherence, or concordance) is a medical term that means the degree to which a patient correctly follows medical advice. Most commonly, it refers to medication, or drug compliance, but may also mean use of medical appliances such as compression stockings, chronic wound care, self-directed physiotherapy exercises, or attending counseling or other courses of therapy. The most effective way for a doctor to improve patient compliance is through a positive physician-patient relationship.

Patient compliance is also increase by:

  • Patient feeling ill
  • Limitations of patients activities due to disease state
  • Written instructions for taking medication
  • Acute illness
  • Simple treatment schedule
  • Short time spent in waiting room
  • Physician recommending one change at a time
  • Benefits of care outweigh costs
  • Peer support

Patients may not accurately report back to health care workers because fear of possible embarrassment, being chastised, or seeming to be ungrateful for a doctor's care. Other causes of poor compliance include:

  • Forgetfulness
  • Poor rapport with physician
  • Few symptoms
  • Chronic illness
  • Prescription not collected or not dispensed
  • Purpose of treatment not clear
  • Perceived lack of effect
  • Real or perceived side-effects
  • Instructions for administration not clear
  • Physical difficulty in complying (e.g. opening medicine containers, handling small tablets, swallowing difficulties, travel to place of treatment)
  • Unattractive medicine, size, shape, or taste
  • Complicated regimen
  • Cost of drugs
  • Low health literacy
  • Choosing to reserve prescription drugs for recreational use or to sell for profit (prescription drug misuse and abuse)

Possession of medical marijuana on private property?
Q. I went to a certain public event with a small amount (about 1gram) of medical marijuana in my pocket. It is mandatory that you be searched for weapons but without my consent the security guard actually stuck his hand in my pocket and pulled out my container labeled "This medicine was produced and exchanged in compliance with CA Prop. 215, SB 420, H&S Code 11362.5 and 11362.7. It can be legally possessed and transported by a qualified patient or primary caregiver." They asked me what it was and I said "Medical Marijuana". I reached in my pocket and showed them my ID card. A. You are allowed to have it in your possession--what part of that don't they understand? Suppose it was some lifesaving or really important drug like nitroglycerin tablets for angina or one of those hypodermic kits people have to carry around who are allergic to bee stings? All you needed was your document (not a medical marijuana card but the form signed by the doctor), and you were allowed to have up to 4 oz. of pot in your pocket (in California anyway, I don't know the laws of other medical marijuana states). They had no right to take it away from you. If I were you I certainly would file a complaint.

A. Thanks for sharing this with the community! I think it's great that we are being allowed to seek alternative care...there are many providers who have been harrassed for practicing reiki and other modalities. If I lived in Texas still (used to live in San Antonio), I'd definitely be there!
